What’s Going On In Vermont
- “Choices for Care” is a Medicaid-funded, long-term care program to pay for care and support for older Vermonters and people with physical disabilities. The Program assists people with everyday activities at home, in an enhanced residential care setting, or in a nursing home.
- “Vermont State Innovation Model [SIM] Grant”: On Feb. 21, 2013, VT was notified of award of a $45 mil State Innovation Model grant from the federal gov’t. This grant will fund activities inside and outside of state gov’t until 2017 to address two core aims: expand and integrate innovative health care provider payment, and expand and integrate health information technology that supports more effective and efficient care delivery.
